For Freshmen
- Entrance examination

- Form 138
- Certificate of Good Moral Character
- Birth Certificate from NSO (photocopy)
For Transferees
- Entrance examination
- One (1) ID picture (1×1, latest)
- Honorable Dismissal
- Scholastic Records / Copy of Grades
- Certificate of Good Moral Character
- Clearance for Admission from the Registrar’s Office
Nursing
The baccalaureate degree program is a four year nursing program, where the clinical duty starts on the second year. It follows the CMO 14, S. 2009 Curriculum from the Commission on Higher Education.
Entry requirements for Freshmen
- Form 138
- Birth Certificate from NSO
- 1 x 1 ID picture (2 pcs.)
- Certificate of good moral character
Screening Requirements for 2nd Year
- At least a minimum grade point average of 2.15
- Passed the Nursing Aptitude Test /Battery Exam (NAT)
- Passed the Psychological Test
- Personal interview
- Possess the Carative Elements of a Health Caregiver
SHS
- Form 138 (Report Card) Original Copy
- Certificate of Good Moral Character from your Junior High School (Original Copy & with school seal)
- Photocopy of Birth Certificate from NSO
- Special Study Permit / Student Visa (for Foreign Applicant)
- Passed TUA-SHS Admission Test
Procedures
SHS
- Secure an application form from the Security Guard, Office of Admission and Registration or download from the TUA website http://www.tua.edu.ph
- Fill out the application form completely.
- Proceed to the Office of Admission and Registration and secure an Applicant Number.
- Pay to the cashier your entrance examination fee.
- Proceed to the Guidance Center and secure your Senior High School Admission Test Permit and Test Schedule or take the test on the same day.
- Results may be viewed from the website: www.tua.edu.ph or you may call 702-2882 locals 172 & 173 after two working days.
